APPOINTMENT POLICIES FOR PROCEDURES AND CONSULTATIONS

Scheduling Format
Procedure appointments are set on a first-come basis. Most doctors’ offices see many patients at the same time, but Dr. Reed sees only ONE patient for several hours. A procedure appointment date and time is reserved exclusively for you with 2-4 hours blocked. Others who wanted that same appointment time are excluded if you booked first. Also, Dr. Reed does not do “robo-scheduling” and strives to accommodate patients’ schedules. Her time is blocked for you, so she needs your commitment to keep your appointment or notify far enough in advance to change it. The same applies to consultation appointments although the time blocked is less.

 Procedure Reservation Policy
To schedule any procedure, a $100 deposit is required ($200 for two procedures). Deposits can be paid by cash, check, credit, or debit card and will be applied toward the procedure fee. The balance of the fee will be due on appointment day. (This policy was necessary due to no-shows who gave credit cards for guarantee that were declined.) At least 48 hours advance notice is required to reschedule your appointment or the deposit will be forfeited. Deposits are not refundable.

Consultation Appointments
A consultation will occur on procedure day for an appointment reserved by deposit. If you desire a separate in-office consultation before reserving a procedure, a $35 fee will be charged. If you schedule a procedure at that consult and pay the deposit, the consultation fee will be included (waived). A separate in-office consultation lasts 30-40 minutes and does not include any makeup designing. Note: All permanent makeup correction or removal cases require an in-office consultation before a procedure will be scheduled. If Dr. Reed refers you to another specialist, the $35 fee will still be charged.

Consultation with Makeup Design Appointments
All makeup procedure appointments include custom makeup designing with your approval before tattooing begins. If you want to “test drive” an eyebrow, eyeliner, or lip design before your procedure, this appointment is your “safety net.” This separate appointment is scheduled prior to your procedure and lasts 60-90 minutes. Dr. Reed will measure and analyze your face and apply conventional makeup to imitate your permanent makeup design. Fee is $95 for one area only (e.g. eyebrows). If your procedure is scheduled within 30 days of the design appointment, $20 will be applied toward your procedure deposit.

Returned Check and Refund Policies
There is a $40 handling fee for any returned check. The results of any elective cosmetic procedure, including permanent cosmetics, cannot be guaranteed. Therefore, whether or not a patient is satisfied with her/his results, there will be no refunds.

 Office Policies on Children and Visitors
Dr. Reed’s office is not equipped for children or babies, therefore child care arrangements must be made prior to all appointments. Adult family or friends may remain in the waiting area if desired. Health Department guidelines do not allow observers in the procedure room. Idaho law prohibits the tattooing of persons under age 18.
